Laparoscopy

Laparoscopy is an operating technique that enables surgery to be performed with the stomach closed, i.e. without opening the stomach wide.

An optic is inserted through the navel.

Air is blown into the abdominal cavity to create a space (pneumoperitoneum) for panoramic vision.

Instruments are introduced into the abdomen through small 5 mm holes in trocars.

Indications

Most gynecological procedures can be performed laparoscopically.

A distinction is made between operative laparoscopy (surgery is performed as if the belly were open) and diagnostic laparoscopy (which enables us to look inside the belly).

This procedure is performed under general anesthesia.

The images are recorded on a screen located at the patient's foot, known as the video column.

Laparoscopic surgery can be used for a wide range of procedures:

  • Removal of ovarian cysts (cystectomy), adnexa (tube and ovary: adnexectomy)
  • Tubal ligation (tubal sterilization)
  • Treat pelvic infection (ovarian abscess, pyosalpinx)
  • Release adhesions in the abdomen (adhesiolysis)
  • Treating a pregnancy outside the uterus (ectopic pregnancy) (photo)
    removal of a fibroid from the uterus (myomectomy)
  • Prepare or perform uterine ablation (laparoscopic or laparoscopic hysterectomy)
  • Treating endometriosis
  • To treat certain cancers, or part of the treatment of certain gynecological cancers.
  • Treating genital prolapse: promonto-fixation.

Diagnostic laparoscopy allows us to search for the causes of problems when examinations do not allow us to be certain, or when they suggest a diagnosis.

  • It is useful in cases of unexplained abdominal pain (adhesions, utero-annexal infection, endometriosis, etc.).
  • To check all the reproductive organs, when there is a fertility problem, and in particular to ensure tubal permeability (laparoscopy with blue test).

Advantages, disadvantages

Advantages : No large visible scar, reduced risk of eventration or hernia, quicker return to activity and less post-operative pain are the classic advantages of laparoscopy.

Disadvantages : In certain situations, the laparoscopic approach cannot be used. Certain procedures, especially in oncology, are not accessible to this type of approach.

In case of operative difficulty, laparoscopy cannot be performed, but will be transformed into conventional surgery (laparotomy: classic opening of the abdomen).
