Cervico-vaginal smear

The cervico-vaginal smear is a routine screening test for cervical cancer.

This examination involves removing superficial cells from the cervix. It is performed during a gynecological examination.

The sample is then examined under a microscope in a cytology laboratory.

Depending on the appearance of the cells, we can assume that the cervix is normal, or that it has an infection, precancerous lesion or cancer.

Most lesions detected by Pap smears are linked to HPV infection.

Indication

The HAS (haute autorité de santé) in France currently recommends a cervico-uterine smear test (FCU) every 3 years (after 2 normal FCUs performed 1 year apart) from age 25 up to age 65 in asymptomatic women who are or have been sexually active, whether or not they have been vaccinated against HPV.

This frequency may be increased if the doctor deems it necessary.

However, it is still possible for doctors to perform a cervico-vaginal smear before the age of 25 in the special case of patients who are at high risk because they had their first intercourse at an early age and have many sexual partners.

Pre-cancerous lesions (cervical dysplasia) have no symptoms.

The only way to prevent them from developing into cancer is to screen and treat them.

Routine smear testing has significantly reduced mortality from cervical cancer.

A Pap smear can be taken during pregnancy.

Even if it causes slight bleeding (the cervix is highly vascularized during pregnancy), it does not increase the risk of miscarriage.
